Through our patient assistance grants, we offer a fast, reliable way to get help paying for medication costs, copays, health insurance premiums, and transportation expenses.

Announcement: Changes to PAN grants

In response to the new Medicare Part D reform that will place a cap of $2,000 on prescription out-of-pocket costs for all Medicare Part D drugs starting in 2025, we are adjusting our grant amounts beginning on January 1, 2025. We design our grants to cover 100% of out-of-pocket costs for most patients—and that isn’t changing. Learn more about this adjustment.

Types of grants

How our grants work

We provide grants to help pay for medication copay costs, health insurance premiums, and transportation for a 12-month period, in full compliance with all relevant laws, regulations, and government guidelines. The amount of assistance you receive, and eligibility requirements, vary by disease fund.

As a patient, you can apply for a PAN grant and start using your grant right away when you’re approved. Your grant funds are yours, and you can choose when and where you use them for your covered medications, regardless of which healthcare professionals are managing your treatment. Providers and pharmacies: you can apply for PAN grants on behalf of patients and help them manage their grants. We provide all the information you need about enrolling patients, helping patients with wait lists, managing claims and billing, and more on our website. Eligibility

Grant amounts and eligibility requirements vary by disease fund. Search our disease funds to learn about eligibility criteria, what is covered, and check your eligibility with our pre-screening tool.

Fund status

  • If a fund is available, it will show as “open.”
  • If a fund is not available, it will show as “closed.”
  • If a fund is available only to people on the wait list, it will show as “wait list only.”

How to apply

Applying for copay and health insurance premium grants

Step 1

Find your disease fund and check your eligibility with our easy pre-screening tool.

Step 2

If you have previously applied for a PAN grant or joined one of our disease fund wait lists, you can apply online or by phone. If not, please contact us at 1-866-316-7263, Monday through Friday, 9am to 5:30pm ET, to apply quickly and easily by phone. Healthcare providers and pharmacies can apply online or by phone on behalf of their patients.

Be prepared with this information for your application:

  • Your diagnosis and medications
  • Your health insurance carrier and insurance member ID
  • Your personal and contact information, including date of birth, mailing address, email address, and social security number
  • Your adjusted gross annual household income
  • Your provider’s contact information, including name and phone number

Applying for transportation grants

You must have a current copay or health insurance premium grant to be eligible. There are two easy ways to apply for transportation grants: check your eligibility and apply online through your PAN portal account or call us at 1-866-316-7263, Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.

Wait lists

If your disease fund is closed, you can sign up for the wait list. When the fund opens, we’ll contact you right away.

Before signing up for the wait list, check the fund’s eligibility criteria to make sure you qualify. The wait list does not guarantee eligibility, so checking your eligibility in advance can avoid delays when the disease fund opens.

You can also sign up for a FundFinder account. Making an account lets you browse a list of over 200 financial assistance programs, including the PAN Foundation. You can then follow any fund you want to get notified as soon as assistance is available.
